2018 Toronto municipal election proxy certificates available starting tomorrow
Eligible electors who are unable to vote for any reason on the available voting days, may appoint another eligible elector to vote on their behalf by completing and submitting a Voting Proxy Appointment form.
A voting proxy must be an eligible voter in the City of Toronto’s municipal election and someone who the elector trusts to vote in accordance to their wishes. A person may act as a voting proxy for one person who is not a family member, or may vote on behalf of any or all immediate family members. Immediate family members are a spouse, sibling, parent, child, grandparent and grandchild.
Starting tomorrow, Voting Proxy Appointment forms will be available upon request from the Election Services office. Eligible voters can call 416-338-1111 or email elections@toronto.ca to obtain the form. Voting Proxy Appointment forms are also available in person from 8:30 a.m. to 4:30 p.m. at:
• Election Services, City Hall, 100 Queen St. W., first floor north
• Election Services, 89 Northline Rd.
• City Clerk’s office, Etobicoke Civic Centre, 399 the West Mall, main floor
• City Clerk’s office, North York Civic Centre, 5100 Yonge St., ground floor
• City Clerk’s office, Scarborough Civic Centre, 150 Borough Dr., third floor
The elector must complete the form and provide it to their proxy. The proxy must submit the form in person at one of the City Clerk’s offices for certification along with identification for both themselves and the elector(s) they will be voting for. Proxy forms must contain original signatures and be certified by the City Clerk’s office to be accepted at the voting location.
